Hospital handoff
A nurse records a changed dose in a chart. The receiving shift uses an older task list. A question about the discrepancy goes to a colleague who recently changed roles. The delay triggers repeated requests.
Constructed teaching scenario. It contains no patient observations or clinical recommendations.
Distributed cognition
- Unit of analysis
- The nurse, patient chart, medication list and receiving team together.
- Signal
- A dose and its time move from observation to record to the next shift.
- Boundary
- Include the chart and handoff; excluding them makes the task appear to reside in one person.
- Experiment
- Introduce a confirmation of the recorded dose and inspect where copying error enters.
- Limit
- The navigation model does not estimate medication errors or clinical outcomes.
Transactive memory
- Unit of analysis
- The team and its directory of expertise.
- Signal
- A question is routed to the person believed to know the answer.
- Boundary
- A consultant outside the ward can still be part of the usable knowledge network.
- Experiment
- Keep expertise fixed while making the directory stale; compare routing accuracy.
- Limit
- Knowing who has expertise does not guarantee that person is available or correct.
Boundary objects
- Unit of analysis
- Groups using the same handoff record for different tasks.
- Signal
- Shared fields preserve identity and timing while local notes serve different needs.
- Boundary
- The standard spans the groups; its local use belongs to each group’s practice.
- Experiment
- Increase a group’s influence over required fields and inspect who pays to adapt.
- Limit
- A completed field does not establish shared interpretation or equitable authority.
Operational closure
- Unit of analysis
- Communications assessed under different institutional criteria.
- Signal
- A delayed discharge can prompt questions about payment, legal responsibility and evidence.
- Boundary
- Functional systems are analytical distinctions; people and organizations can participate in several.
- Experiment
- Compare how the same event is processed under different codes and programs.
- Limit
- A clinician can discuss costs and a lawyer can use scientific evidence. The model’s toy codes do not exhaust either profession.
Feedback and delay
- Unit of analysis
- A correction loop between a request, response and later observation.
- Signal
- A missing response triggers another request before the first response arrives.
- Boundary
- Include the queue and response delay to explain repeated correction.
- Experiment
- Inspect outstanding requests before adding another correction.
- Limit
- The supply-chain instrument illustrates delayed correction; its costs and weeks do not describe a hospital.
